OR download the form (in English or Spanish), print and complete it, then mail it or bring it in person to the Board of Elections, 401 State Street, Hudson, NY 12534, Mon-Fri, 8AM to 4PM.
Important: When you complete the form, put your full upstate residence address in Section 8.
In Section 9, put the address where you’d like to receive Board of Elections mail.
The address in Section 9 can be different from your upstate residence address. It should be the address where you most reliably receive mail.
Include your phone number and email in case the Board of Elections needs to reach you.
